Come and join the small but growing Cheshire West Integrated Care Partnership to deliver our exciting and visionary integrated care across Cheshire West. Over the last 24 months we have developed an innovative approach to care which has the full support of NHS and Local Authority partners.

We are seeking to appoint a business performance and programme manager to deliver programmes of work for a new model of care within the Integrated Care Partnership team.

To apply for this position, you will need to be working in any one of the Cheshire CCG’s, West Cheshire footprint of CWP, Countess of Chester Hospital, Leighton Hospital, Cheshire West and Chester Council.

The business performance and programme manager will provide support to the following programmes:

Care Communities Programme with the development of 9 Care Communities across Cheshire West in line with our Model of Care.
Enhanced Community based Services Programme that will increase resources and develop capability of community services to create Enhanced Community based Services; to enable a community crisis response, management of long term conditions and intermediate care functions to support people closer to home or in their community.
To succeed in this role you will have proven business performance and project management experience within either health and/or social care environment and you will be used to working with senior managers, clinicians and stakeholders across the health economy

The role will be on a 12 month secondment with the terms and conditions of the substantive employer, and at equivalent organisational salary grade and will be based in West Cheshire.

The Countess of Chester Hospital NHS Foundation Trust consists of a 600 bedded large district General Hospital, which provides its services on the Countess of Chester Health Park, and a 64 bedded Intermediate Care Service at Ellesmere Port Hospital. The Trust has almost 4,000 staff and provides a range of medical services to more than 445,000 patients per year from areas covering Western Cheshire, Ellesmere Port, Neston and North Wales.
